✔️ How does Arena 10 work?

Vonoprazan is a potassium-competitive acid blocker (P-CAB) that suppresses gastric acid secretion without requiring acid activation. It inhibits the H⁺/K⁺-ATPase enzyme in a reversible and potassium-competitive manner. Due to its strong basic nature, Vonoprazan accumulates at the acid-secreting site of gastric parietal cells and remains there for an extended duration, providing potent and sustained suppression of gastric acid secretion. This results in effective protection against mucosal damage in the upper gastrointestinal tract.

Vonoprazan

✔️ Dosage and Administration of Arena 10

Gastric and Duodenal Ulcer

• Adults: 20 mg orally once daily
• Duration: 8 weeks for gastric ulcer, 6 weeks for duodenal ulcer

Reflux Esophagitis

• Adults: 20 mg once daily for 4 weeks
• Treatment may be extended up to 8 weeks if necessary

Maintenance Therapy for Reflux Esophagitis

• 10 mg once daily
• Dose may be increased to 20 mg once daily if response is inadequate

Prevention of Ulcer Recurrence (Low-dose Aspirin or NSAIDs)

• 10 mg once daily

Helicobacter pylori Eradication Therapy

First-line regimen (7 days):

• Vonoprazan 20 mg
• Amoxicillin hydrate 750 mg
• Clarithromycin 200 mg

(All administered twice daily; clarithromycin dose may be increased but should not exceed 400 mg twice daily)

Alternative regimen (7 days):

• Vonoprazan 20 mg
• Amoxicillin hydrate 750 mg
• Metronidazole 250 mg
  (All administered twice daily)

Use in Special Populations

Elderly Patients

• Use with caution due to possible age-related decline in hepatic and renal function.

Pediatric Use

• Safety and efficacy have not been established in patients below 18 years; use is not recommended.

Renal Impairment

• No dose adjustment required for mild to moderate impairment (eGFR 30–89 mL/min).
• Avoid use in severe renal impairment (eGFR < 30 mL/min).

Hepatic Impairment

• No dose adjustment required in mild hepatic impairment (Child-Pugh A).
• Avoid use in moderate to severe hepatic impairment (Child-Pugh B or C).

✔️ Arena is contraindicated in:

Patients with known hypersensitivity to Vonoprazan or any component of the formulation

Patients receiving atazanavir sulphate, nelfinavir, or rilpivirine hydrochloride

✔️ Pregnancy and Lactation

Arena should be used during pregnancy only when the potential therapeutic benefit outweighs the possible risk to the fetus. Use during breastfeeding is not recommended. If treatment is essential, breastfeeding should be discontinued.

✔️ Precautions and Warnings

• Patients should be closely monitored, and the lowest effective dose should be used.
• During long-term therapy, periodic evaluations such as endoscopy are recommended.
• Maintenance therapy for reflux esophagitis should be limited to patients with recurrent or relapsing disease.
• Dose reduction to 10 mg or discontinuation should be considered once healing is maintained, and recurrence risk is low.
